Applications are accepted on Wednesday between 1 PM and 4 PM only.

Incomplete applications will not be accepted.
Make sure to use your legal name as it appears on your Social Security card.

Along with the completed application, also bring with you:

• Birth certificates of all household members
• Proof of income for all household members
• Court documentation for any child under the applicant's custody
• Marriage Certificate or Decree of Divorcement
• Social Security cards of all household members
• Florida Driver's License or Florida ID cards for all adults (18 years and older)
• A background report will be performed on each household member (18 years and older)
