Applications are accepted on Wednesday between 1PM and 4PM only.

Please contact our office for an application. 

Incomplete applications will not be accepted.
Make sure to use your legal name as it appears in your social security card.

Along with the completed application, also bring with you:

• Birth certificates of all household members
• Proof of income for all household members
• Court documentation for any child under the applicant's custody
• Marriage Certificate or Decree of Divorcement
• Social Security cards of all household members
• Florida Driver's License or Florida ID cards for all adults (18 years and older)
• A background report will be performed on each household member (18 years and older)
